A New Approach to Harm Reduction & Community-Oriented Healthcare

 

Access evidence-based SUD, MAT, and mental health treatment at your local emergency departments via Conejo Health.

Through the SUD Navigator program, Conejo Health and other community-minded healthcare partners aim to transform the way that substance use disorder (SUD) is addressed and treated in California Emergency Departments.

Program Scope

  • Trusted Support from Qualified Professionals

    A SUD Navigator serves as a vital, judgment-free connector between traditional healthcare providers and patients with SUD, many of whom are at risk and/or dealing with underlying mental health issues.

  • Evidence-Based, Stigma-Free Treatment

    The emergency departments enrolled in our Bridge Navigator program view SUD as a treatable medical emergency and have been trained in current MAT (Medication Assisted Treatment) standards.

  • Connected Solutions, Referrals, and Free Resources

    SUD treatment is not something that occurs overnight. To ensure that patients are supported throughout their entire journey, we provide access to a large catalog of vetted treatment options.

  • Compassionate, Community-Focused Care

    The Conejo Health Bridge Navigator Program helps clinicians, nurses, and other hospital staff to effectively assist SUD patients via training programs, harm reduction materials, technical instruction, and other free learning resources.

CADC or CP-C

Certified Alcohol Drug Counselor (3,570 hours of SUD training.) or Licensed Community Paramedic (5,800 hours medical training, including SUD.)

CCAPP CCHW

CCAPP CCHW is the Industry standard certificate to certify Community Health Workers (CHW).

AHA BLS CPR

American Heart Association basic life support CPR, AED, and Naloxone training for healthcare professionals.

MAB

Management of Assaultive Behavior; healthcare professional training for violence prevention. Meets AB-508 and SB 1299 requirements.

CPI

Crisis Prevention & Intervention; best practices for managing difficult situations and disruptive behaviors.

Blue Fire Card

Acute Care Hospital Fire Safety Class required by Los Angeles City Fire.
