Enhanced Care Management
Personalized support for people with complex health and social needs. We coordinate care, remove barriers, and help patients stay connected to the services they need.
Community Health Workers
Our ED Navigators meet patients at the bedside and help bridge the gap between the emergency department and ongoing care. We connect patients to treatment, follow-up services, and practical support after discharge.
Street Medicine
Bringing healthcare directly to people experiencing homelessness, providing medical care, and connections to ongoing services like substance use & mental health treatment, housing support, and basic needs like food & hygiene.
Harm Reduction Supply
We distribute naloxone, fentanyl test strips, safer-use supplies, and other resources while creating a direct pathway to treatment and support when someone is ready.
EMS Navigators
We partner with first responders to connect people with substance use and complex health needs to care directly from the field. Our navigators provide rapid follow-up, treatment connections, and ongoing support.
Conejo Health Youth Program
We engage and empower young people to prevent substance use through civic engagement, education, and community action. Youth build leadership skills, strengthen their communities, and help create lasting change.
